This orientation meeting is for 6th-grade families and NEW to CDS 7th and 8th-grade families. Parents and students will meet in the theater to receive information about the middle school. After the orientation meeting, students will receive their schedules and will have an opportunity to meet their teachers, locate their lockers, and ask any questions. Please do not bring supplies on this day. Students will have an opportunity to set up their lockers with their advisory teacher on August 12th. If you cannot attend the orientation meeting, students will receive similar information, including their schedule, on August 12th, when the entire middle school returns for student orientation.

Math Assignments - Every 6th-grade student should complete the 6th-grade math packet, unless otherwise contacted by Mrs. Patel.
Please complete the even numbered problems in the summer math packet that is appropriate for the class placement for Fall 2024. Each section has some review notes before the questions begin. All work processes need to be shown to get credit. Answer keys have been provided for use if struggling with a problem and need the answer to work backwards. However, all work is expected to be shown. The summer math packet will be due August 15th or 16th depending on when the student's first math class is scheduled. Make sure all work is complete and if there are specific questions they can be asked during the summer work review in preparation for an assessment. Challenge: if all problems are completed and all work is shown, the student will be entered into a raffle for a prize drawing.

Upper School students have the unique opportunity to experience the unforgettable trip to Pathfinder Outdoor Education. Pathfinder delivers fun, challenging adventures that help students (and faculty) learn about themselves, others, and the environment facilitated by extensively trained Pathfinder educators. The activities chosen for this 3-day overnight class trip include team-building initiatives, challenge courses, high ropes, canoeing, a solo experience, and a campfire. WAIVER REQUIRED

We will end in San Juan with explorations in Calle Serra viewing world famous murals and street graffiti and additional explorations. This trip offers a perfect blend of history, adventure, cultural experiences, and is designed to inspire our students as they prepare for their futures.

CDS students will embark on an exciting cultural adventure across Montreal and Quebec, Canada! Students will visit historic sites in Montreal, visit Mount Royal, tour the Notre Dame Basilica, and enjoy outdoor adventures including ice skating and dog sledding! The trip will continue to Quebec with a sampling of traditional French cuisine, Cuivres d’Art Albert Gilles (Copper Museum), walking tours and toboggan slides, along with interactive experiences around the city! This trip offers a perfect blend of history, adventure, and cultural experiences, and is designed to inspire our students as they prepare for their futures. Merci!

We need your help to be sure CDS has the most up to date, accurate and complete contact information for your family.
Over the summer, we will keep families informed via email. Please log into your Veracross portal to make changes in your contact information (email, mobile phone, or mailing address).

Carrollwood Day School partners with Magnus Health to collect and store your student’s health and medical information as well as required school forms for the upcoming school year.
2025-2026 Required Forms and Instructions
Be Prepared!:
- Check requirements on Magnus before any doctor appointments to download and print all forms requiring a physician’s signature. These include forms for students who will need to take medication during the school day or have a special medical condition as well as the FHSAA EL2 Preparticipation Physical Examination form for athletes in grades 6 - 12.
- Please note: New Families: Your physician will provide the school entry physical form (FL DH 3040) and immunization form (FL DH 680). [Florida Immunization Requirements] The school entry physical form (FL DH 3040 is a two-sided form. Parents must fill out one side and the physician will fill out and sign the second side. Be sure to upload both pages of this document.
- Please note: The Over-the-Counter Medication Form must be downloaded and signed by the physician and parent each year.
How to submit required forms:
- Log into your Veracross portal and click the Magnus Health link on the main page.
- You will see your child(ren) listed on your Front Desk screen. Click on the "Complete Now" button for each child.
- You will advance to a screen with initial "yes" and "no" questions about your child's age and activities.
- Once these conditional questions have been answered, the system will generate a customized "To Do" list with all the forms and other required information (like birth certificates and immunization forms) that need to be completed/submitted for your child.
- You must complete all questions on the forms; for example: if you are asked to list all allergies your child has and your child does not have any allergies, you will need to write "None." The system will not advance to the next page if any questions are left blank.
- You do not need to complete/submit everything in one sitting. Magnus Health will track your progress and will send email reminders to you weekly until all required forms and information is complete.
All forms and information must be completed/uploaded to Magnus Health by Tuesday, July 1, 2025.
ECC REQUIRED FORMS
Hillsborough County Ordinance requires that ECC parents must receive a copy of the “KNOW YOUR CHILD CARE FACILITY BROCHURE/FDCH BROCHURE,” "DISTRACTED ADULT" brochure, information on the INFLUENZA (FLU) VIRUS, and the parents are notified in writing of the Nutrition Plan and Disciplinary Policy used by our school. Copies of these documents are available here to download for your review. Parents will sign the Discipline Policy and the Nutrition Plan including acknowledgement of the Child Care Facility and Influenza brochures on Magnus. Parents should download and sign Distracted Adult brochure and bring to the ECC during their Meet Your Teacher meetings.

CDS Uniforms and Dress Code
Carrollwood Day School is a uniform/dress code school for students in kindergarten through high school. The responsibility for assuring students are in the proper dress ultimately lies with the parents and the individual student. The school expects all students to be dressed appropriately on campus. Students may not be allowed to attend classes unless in appropriate uniform. Students who are not in full uniform for the first time will be loaned appropriate dress, if available. The second offense will result in the student’s parents being called.
Risse Brothers is the school uniform provider for Carrollwood Day School. Risse Brothers has a local retail outlet located at 4228 N. Armenia Avenue, Tampa, FL 33607 (phone 813. 282.8338). Risse Brothers' local store has a special department for CDS uniforms where selections can be tried on and purchased. Uniforms can also be purchased online at www.rissebrothers.com.
Risse Brothers is open for in-store and online shopping all summer.
The approved uniform apparel varies slightly for each school level. More detailed information is included under the section of the student handbook for each school division.
General Uniform Reminder
It is the intent of uniform to create a cohesive environment conducive to a high level of learning while promoting a sense of community.

Hair
All students’ hair should be kept neat and clean. Students are required to maintain a school appropriate appearance at all times.
Hats & Hoods
CDS hooded sweatshirts may be worn in school; however, hats and sweatshirt hoods are not allowed to be worn in school buildings. Hats and sweatshirt hoods may be worn outdoors during lunch, P.E., or recess. Students who consistently challenge this rule will not be allowed to bring hats or hooded sweatshirts to school.Their hat or hooded sweatshirt will be held in the office or by the Dean of Students. The expectation is that students will remove hoods, hats, and earbuds while in the classroom.
Jewelry
Any jewelry must be appropriate for outdoor play. CDS will not be responsible for the loss of any jewelry brought to school by students.
Additional Information on Uniforms
- Skorts/jumpers/shorts are to be worn within 4” of the back of the knee.

CDS Uniform bottoms include: Risse Brothers shorts, skorts, pants, and the NEW CDS joggers on the spirit store. CDS black "Patriots" sweatpants will only be allowed on Spirit Fridays.
- Be sure that buttons on shorts, pants, and skorts are secure and that zippers work.
- Students may not wear their P.E. shirt under their uniform shirt.
- Closed-toe shoes appropriate for school activities must be worn each day.
- Students will be advised when they may wear or change into a team uniform.
- High heels/platform shoes, sneakers with wheels (heelies), sandals, boots, and “Crocs” are not appropriate school attire.
- Shirts worn under the uniform top must not be visible.

Any apparel that is inappropriate or distracts from the intent of the uniform is not permitted.
Non-Uniform Dress Code
Students will be notified when there will be non-uniform days. Half-days are not automatic non-uniform days.
- Students may wear solid T-shirts or T-shirts with college or sports team logos. Students may not display slogans, drug-related pictures or music promotions, advertisements, or offensive pictures.
- Cut-off shirts, midriff tops, or tank tops are not appropriate school attire, even on free dress days.
- Closed shoes must be worn.
- Shorts and skirts cannot be more than 4 inches above the back of the knee.
- Jeans, slacks, or long shorts are acceptable. Ripped or torn jeans are not acceptable.
- We recommend that names be written on the inside of the neckband or waistband.
Dress Code Violations
Any student who is out of uniform or who wears an inappropriate form of the uniform will be notified. Consequences will be assigned as appropriate.

Athletics
Students in grades 6 - 12 who will be participating in athletics over including preseason conditioning and practice are required to submit new forms every year. The athletic forms requirements can be found under "Athletics" on the Carrollwood Day School website; forms can also be picked up in the Hayes Family Gymnasium Athletic Office.
***All players must fill out all the required athletic forms. Unless otherwise noted, all forms need to be uploaded to the Magnus Health Portal or app. This includes doctor physicals and the videos. No student athlete will be able to participate until these forms are uploaded to their Magnus Health account. (See below for athletes participating in summer athletic programs.)***
FOR ATHLETES PARTICIPATING IN CONDITIONING AND ATHLETIC PRACTICES DURING THE SUMMER:
- All required athletic forms should be completed prior to reporting for the first practice.
Transportation
Carrollwood Day School offers transportation to and from the Bearss Avenue Campus on a fleet of school owned buses. This service is maintained for the convenience of CDS families. CDS has established bus stops at various locations on each of our bus routes. In addition to the route bus stops, there is a shuttle between the ECC and Bearss Avenue Campus to assist families with children enrolled at both campuses.
For families that do not use the CDS bus service, Carrollwood Day School encourages all parents to carpool whenever possible. In an effort to "go green," the CDS Admissions team is glad to help parents to find carpool partners. Carpool Form

Student/Parent Handbook
It is wonderful to have you as part of our Carrollwood Day School family. The combined efforts of the PTO, the CDS Board, and the school offer many opportunities for families to participate in their children’s school experiences and to enjoy a sense of fellowship with other families.
It will include information about our school and some ideas about how you can help your child have a successful year. The handbook includes school policies and procedures that apply to all CDS students. We ask that you and your students read and review these policies and procedures. It is a condition of enrollment that you comply with school policies.
We look forward to a year filled with exciting educational opportunities and growth for each student at Carrollwood Day School.

As CDS' foodservice provider, FLIK Independent School Dining believes in a holistic approach to nutrition and wellness. Balance, variety, and moderation are always promoted.
FLIK's various signature programs and new promotions are highlighted monthly and focus on important food groups: fruits, vegetables, whole grains, and legumes. All programs are reviewed annually to assess successes and current innovative trends.
